REPORTING FORM
Thank you for reporting your concerns. Please fill out as many sections as possible in as much detail as you can to the best of your knowledge. The information you provide will help us make an initial assessment of the situation and take the appropriate actions.
Anyone who, in good faith, discloses poor practice and/or concerns about a child or adult at risk will receive full support form FIFA, even if the allegations are subsequently proved to be unfounded. Out of respect for the importance of this issue and to encourage honest and effective reporting, knowingly making a false report will not be tolerated and may be a violation of FIFA’s Code of Ethics and other FIFA Rules and Regulations.
Please note that it will take approximately 15 minutes to fill in this report. All reports will be addressed in strict confidentiality.
